Return form crashes -- can't sumbit RMA request

I am trying to return a defective battery from an order I received today. I fill in all information on your RMA warranty form site, and added three pictures. I hit UPLOAD, the files appear to upload, then I get sent to a blank screen -- I can't complete the form submission.
